I was instructed to submit the harmonic generating equipment manufacturer application form according to the harmonic suppression guidelines.

Answer

The harmonics generation device manufacturer application form must be created by the manufacturer and submitted by the consumer when a device that contains a dedicated circuit configuration not defined in the harmonic suppression guidelines is used.
This document is not required for a general-purpose inverter.
